In the past 25 years, theories of mental illness have shifted from blaming mother to blaming the brain. While the prevailing view is that "mental illnesses are medical illnesses just like diabetes, high blood pressure or heart disease," and it's estimated that 30 million people worldwide have taken Prozac, the truth, argues Valenstein, a professor of psychology and neuroscience at the University of Michigan and the author of Great and Desperate Cures, is that we are only at the dawn of an understanding of mental illness. The studies he reviews indicate that a combination of medications and therapy offers the best chance of success at treating common disorders, although no one knows exactly why. Valenstein does a fine job of illuminating the various interests at work behind the ascendancy of purely biological hypotheses. They appeal to pharmaceutical companies, he suggests, for all the obvious reasons, and he details the impact that these companies have, at every level, on today's psychiatric landscape: from sponsoring research and colloquiums to lobbying government to marketing directly to both consumers and primary-care physicians?the largest prescribers of psychiatric drugs. The companies also, he reports, pressure editors of psychiatric journals, in which they also advertise, to downplay studies that cast doubt on the safety or usefulness of their drugs. Families and patients, meanwhile, embrace biological theories because they relieve them of the burden of blame, and physicians, he says, neglect their responsibility to report side effects to the FDA. This meticulously researched, evenhanded work deserves a large audience. Unfortunately, it's about as exciting to read as the fine print in your HMO contract; Valenstein, who comes out with both guns blazing, concentrates more on clearly digesting the data than on giving the story a human face.

Chemical imbalance theory, early on, exposed as bogus.

After Great and Desperate Cures, the story of the rise and fall of psycho-surgery and various shock treatments, written earlier, in fact, the second part of the same story. Blaming The Brain is the story of the rise of drug treatment, and the bogus chemical imbalance theory that accompanied it. Although not the book that Great and Desperate Cures was, definitely 5-star material in my book, still a very worthwhile and informative read. Great and Desperate Cures represents, for one thing, a necessary corrective to a few of the shortcomings of Jack El-Hai's relatively, for lack of a better term, straight forward biography of Walter Freeman, The Lobotomist. Blaming The Brain, while falling prey to a few of the errors of conventional psychiatry, is conscious enough to see through those errors at the same time. Blaming The Brain was published, in the main, before the advent of atypical neuroleptics, however it was also revised in reference to them. This is one of those books that people should read. It exposes many of the myths associated with psychiatric treatment, and stemming from the profession's relationship with the pharmaceutical industry, for the myths that they actually are. While not wholly on the side of psychotherapy, it shows up some of the biases of bio-psychiatry for the biases that they are, and with them, exposes some of the bad science behind it.

Psychicatric Madness

Elliot Valenstein's book is at the same time informative, provocative and disturbing. He has analyzed a dense literature and distilled from it key ideas that run counter to the current dogma of psychiatry and other "helping professions," the currently fashionable view that so-called mental illnesses of many types are brain disorders. Valenstein prepares the reader with a well-crafted history of biological psychiatry, followed by a knoweldgable and intelligent critical analysis of the literature. This is a book that deserves a wide readership but, alas, will probably not receive it. The juggernaut of the brain psychiatrists and their sympathizers is just too overwhelming to give Valenstein's book the careful reading it deserves. I look forward to his next work, which I hope would examine the related claims for a biological basis of ADHD, homosexuality, alcoholism, and the like.

Silver Bullets and Free Lunches

Once or twice a month, in many psychiatric hospitals, researchers present data showing the therapeutic efficacy of a new drug (or tweaked older one with a new label). The charts and graphs about these "silver bullets" usually feature percentages of psychiatric patient improvements over six to eight weeks in comparison with those treated by placebo or competing meds. The sample sizes are typically small and, at least in the many of these presentations that I attended, even the simplest descriptive statistics (means, standard deviations, etc), to say nothing of measures of sample overlap (Cohen's d scores) and meta-analyses, are nowhere to be seen. Nor are they readily available from the presenter. The attending psychiatric staff sometimes raise questions about the area of the brain or nerve receptor the drug targets while they enjoy the fine and plentiful free lunch provided by the sponsoring pharmaceutical company. It would be difficult to conclude other than that issues of empirical validity had been comfortably settled long ago. Thus, these concerns were far beyond the mattering maps of the audience. An earlier generation's favored cure was lobotomy before, in the early 1950s, the discovery of Thorazine's (chlorpromazine) quieting effects ushered in this, now dominant, psychiatric treatment paradigm.

Elliot Valenstein's BLAMING THE BRAIN: THE TRUTH ABOUT DRUGS AND MENTAL HEALTH demonstrates why rationales for this paradigm ain't necessarily so. Valenstein, professor emeritus of psychology and neuroscience, University of Michigan, continues the program begun in his GREAT AND DESPERATE CURES: THE RISE AND DECLINE OF PSYCHOSURGERY AND OTHER RADICAL TREATMENTS FOR MENTAL ILLNESS (HarperCollins, 1987) and now followed by his most recent THE WAR OF THE SOUPS AND THE SPARKS: THE DISCOVERY OF NEUROTRANSMITTERS AND THE DISPUTE OVER HOW NERVES COMMUNICATE (Columbia Univ. Press, 2005). He helps his readers look beyond the shiny surfaces of therapeutic regimes and into the empirical heart of these matters. In BLAMING Valenstein draws our attention to the transition in psychiatric treatment ideology from mothers as causes of mental illnesses to the current conventional certainties about chemical imbalances in the brain, which he says, though in other words, rest solidly on a foundation of sand. He describes how neuropsychiatric theory got from there to here, then to now, jettisoning untidy, equivocal empirical data along the way.

In BLAMING, a rich historical context is provided to make sense of the scientific, social and economic forces that led to the now, largely unchallenged, happy and enduring marriage between the pharmaceutical industry and the psychotropic treatments that identify most of present-day psychiatry. [See Peter Breggin's TOXIC PSYCHIATRY, St. Martin's Press, 1991 and C. Ross & A. Pam's (Eds.) PSEUDOSCIENCE IN BIOLOGICAL PSYCHIATRY, Wiley, 1995 for notable exceptions in the psychiatric community]. Included on the theoretical side are the speculations of the 19th century figures like Thidicum, the founder of modern neurochemistry and Freud, both of whom suggested a neurochemical basis for mental illnesses, and the chemical elaboration of LSD accompanied by the English professor of pharmacology, John Gaddum's suggestion that LSD-25's effects might be caused by blocking the neurotransmitter serotonin, a newly discovered brain chemical he thought essential for balanced mental activity.

On the treatment side, Valenstein traces the emerging trials in the early 50s with Thorazine, initially marketed as a surgery anesthetic. This led to it and later generations of drugs becoming the treatments of choice for the schizophrenias. Similar development pathways are described for the anti-depressants and anti-anxiety meds. An anecdote not included, but that gives some idea of the patient's experience with these "tranquilizers," is the story of Dr. Cornelia Quarti, the psychiatric resident who submitted experimentally to a trial with Thorazine in the earliest days of its use. As the drug took effect she tried to shout out, "No! You are bothering me!" and tried to fight off the subsequent feeling of imminent death with all her energy. Later, she listened in amazement as the experiment tape recording revealed, a "weak and monotonous voice..." [cf. D. Cohen in S. Fisher & R.P. Greenberg (Eds.), FROM PLACEBO TO PANACEA, Wiley, 1997].

Professor Valenstein's treatment of the empirical and theoretical developments in neuroscience and psychopharmacology is detailed and exhaustive including issues surrounding electrical versus chemical stimulation of the brain (the Sparks and the Soups camps), relationships between the discoveries of neurotransmitters, brain centers for reward, eating, rage and physiological concomitants of emotion (sweating, changes in heart rate and blood pressure, etc.), and biochemical theories about how these might explain the effects of psychotropic medications and account for mental illnesses. Not the least of these accounts were theories of schizophrenia (excess dopamine) and depression (e.g. serotonin deficiency) that are still favored in the trenches of clinical psychiatry. For reasons that are carefully explored in Chapter 4 (A Closer Look at the Evidence) and Chapter 5 (The Interpretation of the Evidence), these and other biological theories are critiqued on grounds that are logical (cause/effect confusion, reductionism), empirical (brain plasticity in response to experience; complex, often indeterminate, disease pathways) and moral (political/economic forces through which marketing, corporation earnings and bare bones state hospital budgets coalesce in spite of the known limits and iatrogenic effects of sustained treatment with psychotropic meds and proven psychosocial community treatment). See Chapter 6 on the pharmaceutical industry influences and Chapter 7 on special interest groups.

BLAMING provides a thicket of facts woven together clearly, crisply and expertly. Following the evidence, Valenstein comes down hard on current treatment with psychotropics, biological psychiatry and the partnership between Big Pharma and medicine. He makes clear, there is an important place for medication. However, the complex interplay between biological and psychosocial phenomena means that the silver bullet, the wonder drug that can solve the lock and key problem of mental illness, or psychotropic cocktails, are unlikely fixes for problems of living. And, no lunch is free as medical staff surely ought to know.

INTRODUCTION: LETTING OUT THE SECRETS

One day the news of an impending hurricane went sweeping through a seaside town. As the winds were picking up and the tide rising, the authorities rushed from door to door, instructing the residents to evacuate, to pack up their valuables and head inland. All of the townspeople -- except for one -- did as advised, acting as fast as they could, loading their cars with their family photographs, TVs, clothes, cash, and their most important records and documents. Most were well inland by the time the torrential rains began hitting their town. Only one man stayed behind, saying calmly to the authorities as they knocked on his door, "No, I'll be okay. God will take care of me."

As the town started to flood, with water swirling waist high, a boat with a rescue team made its way to the man's house, urging him to climb on board and get to safety. Again the man said confidently, "No, I'll be okay. God will take care of me."

Soon the rising waters submerged almost all the houses in the town and the man found himself stranded on his rooftop. A Coast Guard helicopter appeared, and lowered a ladder for the man to climb to safety. Again he refused, calling out, "No, I'll be okay. God will take care of me." It was his last chance. Within an hour the floodwaters engulfed the house, drowning the man.

Upon reaching heaven, the man, totally confused and feeling utterly betrayed, appeared in front of God and asked, "Why did you not take care of me and save me from the storm?" God replied, "I did try to take care of you. I sent you a messenger, a lifeboat, and a helicopter! What more did you want me to do?"

What more, indeed?

FIVE TIMELESS LAWS OF MONEY, FIVE SECRETS

When I travel the country, I am saddened to see the faces of all the people who have gotten into serious financial trouble. They are smart people, good people, kind people. So what happened?

The answer is really very simple. These people, just like the man in our parable, did not know the financial signs they needed to pay attention to when they appeared in front of them. They did not know what to do with their money and when to do it.

Let's get real; the person who I am talking about is you. You -- like most people -- don't even like to think about the financial what-ifs of life, let alone talk freely about your money. You, like many people, probably feel that the topic of money is so personal that it needs to be kept tucked away like a deep dark secret. And because you treat money as a secret, you don't have the opportunity to discover the Laws of Money.

And that is where your trouble begins: Even though you don't know about a law, you can still end up breaking it. And your ignorance of the law doesn't get you off the hook. You still suffer the consequences if you break it.

Think of a well-known law that is posted and straightforward -- for instance, the highway speed limit is 55 miles per hour; that's the law. You know very well in that case when you are breaking the law and when you are not, for there are reminders of that law every few miles. That is why when you are speeding and breaking the law, you are constantly looking in the rearview mirror to make sure you do not get caught. At least in this case you are aware that you are breaking the law, and if you get caught, you have no one to blame but yourself.

But when it comes to money, there are no huge signs constantly posted along your path to tell you how fast to go, when to yield, or even when to stop. So you are often breaking the laws of money and just don't know it. But, even worse, you have no idea of the consequences you will suffer if you break a law of money. And when you do break a law of money, the end result makes you feel as if you are in financial prison -- you have less money, more debt, and no security. When you know the laws of money and follow them, however, you will be able to create more money, get out of debt, and increase your personal freedom.

DON'T GIVE UP -- YOU WILL DO IT!

So many people have been greatly affected by the volatile economy of the recent past. It's likely you have, too. Maybe you have worked hard all of your life and now, just as the promise of retirement approaches, find that much or all of your money you so carefully saved has disappeared. Maybe you invested your time and money in what you thought were no-lose propositions, but you did lose -- big-time; it may take you years to recover your losses and years more to get ahead. Perhaps you lost your job and are having trouble finding another. Or possibly you are just simply overburdened with mortgage debt, car loan debt, credit card debt or student loans, and you don't see a way ever to get above water; your financial stance is, Why should I even try?

This is what I want you to remember: Every person I talk to who is in financial trouble would have benefited from knowing the five laws of money that I reveal in this book. Even though every person's financial situation is unique, the laws of money are universal. Even if you have made some bad investments or gotten in over your head with your credit cards, these laws will lead you back to a trouble-free plan for your money. All you have to do to start is to read this book from cover to cover.

Is it really that easy? You bet. Listen, there will be many times in your life when you look back and say, "If I only knew then what I know now...." But when it comes to money, I believe from the bottom of my heart it does not have to be that way. Good advice, even if not original

Suze Orman has offered advice for years and most of it is sensible basic stuff. This time around she offers 5 basic principles (she calls them 'laws') about money. These laws are right on the money and apply to almost every reader. Adopting these laws will definitely lead the reader to a more satisfied financial life. The second part of the book (NOT written my Ms. Orman) is a guide enabling the reader to make these laws part of his own life. The very first law is to be truthful about money and to stop lying to yourself and others. For example, if you drive a more expensive car than you can afford, or if you dress in the latest expensive styles, even though they are beyond your means, or if you pile up credit card debt to buy items merely to impress others, then you are lying to both the outside world (pretending you can afford those items) and to yourself (telling yourself it is ok to live on borrowed money). Coming to terms with those lies, being truthful and living within your means is the road to avoiding financial misery. This lesson is not new, but Orman's presentation is. The message is powerful and presented well. Some might try to compare this writing to that of Kiyosaki, but that is unfair. Kiyosaki's writings are inspirational and uplifting, but short on real advice. Orman's latest book is not only insightful, but offers good advice and the guide is helpful in getting the reader to follow the advice.

Most credible financial author today

Not only is Suze Orman the most listened to financial author today, she is also the most credible and widely trusted. In her newest book, "The Laws of Money, The Lessons of Life Suze Orman explains not only the "how to's" of personal money management, but also the emotional issues that inevitably accompany any effort to get one's financial house in order.This is an excellent book and should be read in addition to "The Road to Wealth" and "9 Steps To Financial Freedom."You've read the rest, Suze is the best.

The Laws of Money, The Lessons of Life

Suze's newest book The Laws of Money, The Lessons of Life is different to say the least. In this book, Suze gets inside your head and uncovers the real reasons why you are not where you want to be financially.
Suze explains why your ideas about money and in particular, your feelings regarding your personal experiences with money play a major role in what you do with money in the future.
A lot of people got wallopped in the stock market over the last three years. This is unfortunate as there are strategies that could have helped these people make omney while the markets went down. Back to the point of Suzes book, many people are now gun shy of the markets and afriad to invest. Big mistake!
The same is true with other areas of personal finance. Some have gone over their heads in credit cards debt and now look at anything plastic with a phobic-like reaction. Others have lost their homes in foreclosure and many have gone bankrupt. Too many others are living high while investing less and less. Contributions to 401 (k) savings plans has decreased and only in January of 2004 has investing in stock mutual funds started to rise.
Too many people are missing out on opportunity. Suze's book will help break that phobia and achieve the results you want.
Fans of 1,000 page boring and out-of-date financial books written by financial authors may not like this book by Suze but I have a question for you: how is the advice that you recieved from that magazine author working for you? I rest my case!
Suze may not be the greatest writer in the world but she is the best financial advisor/author right now. She is genuinely passionate about personal finance and not just trying to catch the wave of personal finance interest and jump on the bandwagon of Venita Van Caspel, Charles Givens, Terry Savage, or now Suze Orman like another personal finance author.
The Laws of Money is a great book that can and will make a significant differnce in your financial life.

From 1866 through 1969, the Hawaiian and American governments banished nearly 9,000 leprosy sufferers into exile on a peninsula on the Hawaiian island of Molokai. Former Outside editor Tayman crafts a tale of fear, endurance and hope in telling the story of these unfortunate victims of ignorance (leprosy is caused by a simple bacteria and isn't nearly as contagious as was long believed). After a smallpox epidemic wiped out a fifth of the Hawaiian population in the 1850s, leprosy was seen as the next cataclysmic threat, and drastic measures were taken. For more than 100 years, anyone diagnosed with the disease was taken to the remote colony. Initially, conditions were horrible, with few services or proper medical treatment. Pushed to their limit and fueled with potent moonshine, the internees frequently rioted, causing overseers to enforce cruel laws. Later, as science and social thinking evolved, conditions improved and many in the settlement lived lives of near normalcy. Drawing on contemporary sources and eyewitness accounts of the still surviving members of the colony, Tayman has created a fitting monument to the strength and character of the castoffs in particular, and human beings as a whole. Hawaii's isolation from foreign illness slowly disintegrated through the nineteenth century as trading ships arrived bearing the yellow flag of disease. When leprosy cases appeared, panicked local officials designated the island of Molokai, some fifty miles from Honolulu, a "leprosarium," because it was naturally inaccessible, presenting a sea cliff "so sheer that wild goats tumbled from its face." The first twelve lepers were rowed to its rocky shores in January of 1866. Drawing on eight thousand pages of documents, Tayman reconstructs a fascinating history of the settlement, which officially lasted until 1969. Shortages of food, water, and shelter sent some lepers into caves pocketed inside an extinct volcano. Tayman's multilayered account sketches in scientific details, such as the fact that later medical studies proved that most of the exiles weren't even contagious.

*Starred Review* Drawing on letters, journals, newspaper articles, medical documents, interviews, and other source material, Tayman has crafted a gripping history of the leper colony at the Hawaiian island Molokai. In the mid-1800s, when fear of leprosy spreading throughout the islands reached a fever pitch, Dr. William Hillebrand suggested the idea of isolating those infected from the rest of society. The colony opened in 1866, and after a stint at a hospital in Honolulu, patients were rounded up and shipped out to Molokai on a regular basis. The beginning years were especially hard, as patients were thrust into the settlement with inadequate supplies and no medical care to speak of. As the colony began to grow, it garnered attention from around the world, and missionaries such as kindhearted Father Damian came to bring religious services to the patients and writers such as Robert Louis Stevenson and Jack London traveled to Molokai to chronicle the plight of the colonists. As Tayman's narrative broaches the second half of the century, the accounts become more personal, culled from interviews with elderly patients who were originally sent to Molokai as children. Tayman's crisp, flowing writing and inclusion of personal stories and details make this an utterly engrossing look at a heartbreaking chapter in Hawaiian history. Kristine Huntley

Uneven

It's hard to imagine the devastation that took place over 2 generations in Hawaii. The population was decimated by diseases for which there were no known causes and no cures. With the emergence of yet a new disease, one with very visible effects there should be no surprise about the panic it caused.

The story of the first exiles is heartbreaking. Their exodus was so poorly planned that the first patients (all quite ill) once disembarked, had to walk up a cliff and then further to a new "home" on which they were expected to forage and farm and build their homes. The captain of the boat that brings these and subsequent exiles to Molokai escapes incarceration for obvious and continuing acts of theft and abuse of the exiles, a true metaphor for the neglect of the Board of Health. From forboding beginnings, a thriving community emerges and later wanes due to advances in medicine.

I think this book was rushed to publication. While the author did 18 months of research, I don't think he digested it all. The result is a book that doesn't hang together very well. It seems that after finding a lot of interesting information Tayman wanted to use it all whether or not he could provide context for it. Where he was able to produce full stories, I'm not sure he had a handle of their significance. I'm not critical of this because I understand their significance, but I expected the book to be more than collection of anecdotes and facts. This book has a lot of very good information, a lot of which I believe is newly presented, but I think the author needed more time to reflect on the topic for a more coherent presentation of it.

Some things are not followed up on, like the personality of Ferdinand Hutchinson, who seems to be focused on doing things cheaply. Even before his exiled son is introduced, he is dropped from the narrative (How responsible was he for the boat captain? What was his attitude/political posture towards his son's exile and did his views change upon it?) Walsh's administration is portrayed as a failure, but later a rather large hospital built in his tenure is casually mentioned. There are some places which I think would be important turning points, but little information is given. It would seem that the introduction of water and electricity would be a major sociological changes for the residents, as well as logistical achievement of the Board of Health, but it gets only a passing mention. Taymen cites things that improve after the death of King Kalakua, and in 1907 Jack London praises the settlement. How these changes occurred and who was responsible for them is not clear from the text.

There are some strange adjectives, for instance "The Little Grass Shack" is labled a "nonsensical" song. There are vignettes that cause pause, for instance during WWII, a civil defense officer has boys march with sticks "hoping enemy submarines will mistake them as soldiers." This one sentence is planted in the middle of a narrative on how the area was being protected. Could this be literal?

This book has caused controversy in Hawaii. The cover photo is allegedly a cliff in Italy. There is criticism of the title. More important, the survivors, who are profiled in the last chapters, perhaps 20% of the book, have objected to their portrayal. Tayman writes sympathetically of them, but some of his phrasings could be construed as condescending to them. One portrait even begins with his minimization of the endearing, "Grass Shack" song.

I enjoyed the book but I have my doubts

"The Colony" would appear to be well-researched (the citations are extensive and detailed, and occupy nearly a fifth of the book), and sympathetic to the patients at Kalaupapa. Certainly it is well-written & interesting. I was prepared to give it a glowing review, but troubling details began to peek out at me.

As I perused the notelines, I observed that two of the four patients whose stories occupy the latter half of the book had withdrawn their cooperation from John Tayman's project. A third patient, Bernard Punikai'a, did not participate (so the notes say) because his poor health prevented him from doing so. Most of his story comes from secondary sources.

Tayman did not go into specifics as to why these patients withdrew their support, but a spate of articles (most prominently one in the NY Times) have appeared which charge Tayman with sensationalizing the story of Kalaupapa to suit his own purposes. Bernard Punikai'a may have been too ill to be interviewed, but apparently he was well enough to contact the publisher and give subsequent interviews expressing his disapproval of being included in this book without his permission.

I can't help wonder why the other patients (there are more than four surviving patients) were not interviewed or profiled...Tayman is unclear on just who all was interviewed, but I suspect that other patients may have smelled a rat and chose not to cooperate.

Some of the articles I read indicate that Tayman consents only to answering questions other than by e-mail. This sounds more than a little fishy to me. Is he afraid of getting caught dissembling?

All this is a shame, because the book at times does a very good job of examining the tragedies concerning public policy towards leprosy. While Hansen's disease now is largely controllable and can no longer be perceived as a public health threat, Tayman draws parallels between the stigma then and the current stigma of AIDs patients. Same story largely, but a different illness. I suspect that he could have drawn these parallels equally well without raising all of the ill will that he has.

In any case, approach this book with some caution. Read the reviews and the articles (notice that the reviewers from Hawaii, who would know the story of Kalaupapa better than I) are almost uniformly harsh in their assessment. Keep these criticisms in mind as you read this book, and remember that there is more than one version to this story.

Inspiring!

An extraordinary story of the unfortunate men and women who were torn from their families and homes, and then banished to the leper colony at Molokai. I came away with immense awe and respect for each and every patient forced to endure this terrifying exile. It's a harrowing tale, but the author uses historical perspective, detail, and even humor to tell it. The book also highlights the courage and resilience of these people. Highly recommended.

Attaining and then maintaining religious sense or persuasion is often difficult today. Our culture of individualism, self-sufficiency, and competitiveness thwarts, even nullifies spiritual inclinations, with technology a prime contender for our reverence even though it is totally "witless and unimaginative" on its own. Yet many people are vaguely aware of something lacking in their lives. Rabbi Kushner (best known for When Bad Things Happen to Good People ) believes that "human life has meaning . . . but only in religious terms." According to this crucial realization, it is religion that connects us to God and community. In the end, Rabbi Kushner goes so far as to define religion as community rather than theology--a point of contention. What, then, would be the point of his title? Excellent book

This book hit the spot for me: a believer who is not sure of the details of my belief and leery of organized religion. Kushner addressed all of the concerns I've been wrestling with on my own, specifically regarding the corruption, bullying, small-mindedness of many churches, including the common belief that only they have the truth and everyone else is on the fast track to hell. I have always been driven to find a religious community but, frankly, it's hard when you're a thinking person, and Rabbi Kushner offered reasons for both why the search is beneficial and how a person can reconcile herself with an extremely imperfect church without necessarily having to agree with or condone the imperfections. Why would a person do that? What's the benefit? He offered plenty of excellent points, and, mind you, without any overt Jewish bias. As a gentile, I really appreciated that. I also appreciated his view that the specific details of theology or doctrine don't necessarily define a religion or make it true or untrue. If it's helping people grow and change, it's true in an essential way that can't be proved factually or empirically. That was also Joseph Campbell's feeling: religions are a chosen manner of connecting to the great unknown, and, in that way, they're all true.

King's epical Dark Tower hastens to a close, and its penultimate volume is one of the speediest. The gunslingers of Mid-World and other alternate Earths have defeated The Wolves of the Calla (2003) but lost one of their number. Susannah Dean, nee Odetta Holmes, lacking her lower legs after a minion of the Satan of Mid-World, the Crimson King, pushed her in front of a subway train, and whose personality is sometimes split between black bourgeoise Odetta and viciously paranoiac Detta Walker, has been taken over by the spirit Mia to be the body in which Mia will gestate a boy who will eventually kill head gunslinger Roland. The child is to be born in New York in 1999, which is where Susannah-Mia repairs through one of the doors between worlds. The other gunslingers pursue through the same door, but only 11-year-old Jake Chambers, accompanied by former 'Salems' Lot priest Don Callahan, get to New York. Roland and Susannah's husband, Eddie Dean, tumble into an ambush in New England in 1977. Each chapter--called a stanza and ending with two songlike quatrains--advances one subset of gunslingers' progress. King keeps us on tenterhooks throughout--and leaves us there. If you thought that the previous five Dark Tower books were departures for King, just wait until you read this one. At some point around the 11th Stanza ("The Writer"), "Song of Susannah" is, like the song says, guaranteed to blow your mind.
Picking up right where "Wolves of the Calla" left off, our heroes Roland, Eddie, Jake, and the relative newcomer Father Callahan prepare for yet another sojourn out of Roland's world and into ours. Right off the bat, though, things do not go quite as planned. A Beamquake shakes the foundations of all the worlds, and we learn that the Tower is in much greater jeopardy than we may have previously suspected. And as always, wherever Roland goes, gunplay is sure to follow, but this time, it's waiting for him...
King's further explorations into the rich world of the Dark Tower are as rewarding as they ever were. The characters, by now, have become as comfortable as old friends. Still, there are new facets to be seen yet, and we get a closer look at each of them as the story goes on.
It's very difficult to write about this latest installment without giving what makes it so different away completely. The events and revelations found in "Song of Susannah" are so central to the themes of the overall story, and yet revealing them here would entirely ruin the fun of discovering them as King has presented it. Some readers will doubtless dislike the road King has begun to travel as the story approaches its conclusion, but I am convinced that many more will absolutely love it. It is a credit to King's growth as a writer that he can even attempt this ambitious sort of storytelling, and more, that he can do it successfully... at least, so far.
One thing is certain: love it or hate it, "Song of Susannah" is King's riskiest and most surprising work yet. One the one hand, he is taking one of the most overused plot elements in fiction -- the baby of uncertain parentage -- and making something original and interesting out of it. On the other, he is attempting something seen in modern fiction only rarely... a self-relexive work that engages the reader on multiple levels. By the end of "Song of Susannah" you may find yourself thinking about the realities that fiction creates, and the fictions that "real" life presents us with every day. And if you do, I believe that is entirely the point. King seems to be angling not only toward a conclusion to Roland's quest, but also toward a deeply personal statement about what it is to be a writer. It is an ambitious road to travel, but so far, King has not let us down. In fact, what he has begun with this book has the potential to exceed all the expectations I had for it.
'Ware, Constant Reader: "Song of Susannah," like "The Waste Lands," ends with a cliffhanger. In fact, there is not only one cliffhanger here, but two. The last pages of "Song of Susannah" should leave many readers, as it left me, powerfully hungry for the final book in the series. What lies in wait on the final page is, to say the least, quite a shock. It leaves the fate of Roland and his companions, as well as that of the Tower itself, enshrouded in doubt.
"One more turn of the path, and then we reach the clearing."
I, for one, can't wait to get there.

Not our Quest, but a fine journey...Are we there yet???

Dear reader, if you've lived through the other 5 installments you must read this one. Period. How could you not?
That said, it is sad to think that this epic journey seems separated into Books 1-4, and Books 5-7 (or at least 5-6, one hopes King jumps back on the right track in 7). Where the first four books were engaging, suspenseful, moving and heroic, Books 5 and 6 seem unimportant and done with much less care.
It's still our favorite crew--Roland at the mast, Eddie and Susannah behind with Jake quickly growing to match them--but they don't have the same spirit. Time has muted their shine. When Roland used to speak, he demanded our attention. I find him withering in these latest books, and not because of his arthritis. What a shame.
Overall 5 and 6 are enjoyable, in and of themselves. As a lover of these worlds, with their strange similarities and inconsistencies, it was great to once again spend a little time with the ka-tet. Yet the Tower's unfolding mystery seems too easily unraveled since the end of Book 4, and without much imagination. King is a gifted writer, to be sure, and occasionally we are flashed with his signature wit and intellect. What we miss are the high stakes that arose from this being an honest tale, with fantastical elements.
Instead we are treated to some gunfire for arbitrary reasons (ka, I guess?) with stock antagonists--and not terribly charismatic ones at that (Andy the Robot? Jack Andolini returns in two more installments?--Stephen, come on, the Lobstrosities had more personality). Most of `Wolves' was a diversion from the tale, detouring to reconnect with Peter Callahan. Slight frustration aside I was fine with that, thinking King was laying some subtle groundwork. Yet in `Song' he continues to wander, and this time THROUGH time (and, apparently, the Northeast).
Much of the story focuses on Susannah's wrestling with her other personality, Mia, hence the title. These two take row after row with one another, battling over her unborn chap. This offers little to no suspense, and the story unfolds in literal fashion, each scene working into the next in an ordinary manner. King connects the dots in lackluster numerical order, quite unlike him, and we miss the Mid-world shocks and specificity he once graced us with. His larger themes are drawing closer together, and--surprise!--we're not in the least surprised by his answers, and nod in polite acknowledgement.
When time and worlds begin to break loose, it's just accepted by the gunslingers--always fascinating that they seem to know both nothing and everything at the same time. Rather than the author spending the time to find complex ways to tie together these loose elements, we are given a premise that is lacking, at best. Readers started to see it at the end of `Wizard,' with his other book references and the like, but it culminates with a trip to Maine where we meet Mr. King himself, one--if not THE--apparent creator of all worlds [while I have a personal (and literary) objection to the author choosing himself as the epicenter of this journey, I must concede it was cleverly crafted, and one of the stronger portions of this book].
Also, King has fallen back to silly pronunciations and syllogisms in this installment, as in `Wolves.' The people speak with stilted and forced language, as if trying too hard to be the characters they once were. There is little ease or subtlety here, and very few surprises that won't make a fan shake his head and say, "No, no, please don't do that, Stephen you're off the Beam." He is successful in providing a cliffhanger ending, and King is always his best when he's in the midst of action. Yet such contrived storytelling on top of the numerous missteps in the last two volumes can leave one fearing the Dark Tower is little more than a mirage at the end of this tale, nothing more than gun smoke and mirrors.
I hold out hope that he is able to return to some of his earlier brilliance, as in `Drawing' or `Waste Lands' for his finale, but even if he continues along this skewed path, it will still have been one heck of a journey. King knows the face of his father, and his work is always worth devouring. This volume, if for nothing more than nail-biting curiosity, is no exception. 5 and 6 are (to me) lesser works in the series, yet it's a good sign that `Song' proved more exciting and a better read than `Wolves.' You might optimistically feel he is wading in the water, keeping you treading just long enough to overwhelm you with one last somersault from under the surface. Ignore the obvious foreshadowing, and you'll find reason enough to keep you looking towards the fall, when (for good or bad) Roland finally ends his journey.
As a last note, the illustrations by Darrel Anderson were mysterious and haunting, a great departure from the uninspired artwork of the last book. Each one delighted, surprised and struck me. Here's hoping `The Dark Tower' does the same, with the striking force of the Gunslinger.

The suspense is terrible...I hope it will last

I have never read so many mixed reviews for a DT book as with this one, and I was at the point where I didn't know what to think about it. Then I read it, or rather FLEW through it, and I think this book is great.
I was never incredibly compelled by Susannah/Detta/Odetta in the previous books but this one changed my opinion of her. Susannah is incredibly strong, and her struggle with Mia is haunting. Mia is actually a very interesting character. I don't fear her, I don't really like her, but I pity her. However, I felt that the Mia's story and the truth of her "chap" was a bit contrived. Especially his name and his purpose (no spoilers here, but if you read it, you may roll your eyes like I did.)
I also love the pairing of Eddie and Roland. I think Eddie makes Roland feel as if he is still a part of his old ka-tet of long ago, and they bring out the strength in each other.
Jake, Callahan, and Oy's mini-tet was also interesting, and I wish there had been more of them in this novel, but I have a feeling they'll hold an incredibly important role in the next book. The change in Jake is fascinating. He knows his purpose now, and he is no longer a boy. He has been to war and seen death and he is hardened but not emotionless. Also, I am incredibly attached to Oy at this point and there is a scene where he starts to cry when he thinks Jake is going to leave him behind and I felt a little tear trickling...then felt a bit silly. And Callahan has been well developed in the short time we've known him.
I think that King's inclusion of himself in this book was brilliant. I can't wait to see what role he'll play in the last novel. A little self-indulgent? Of course! He's a WRITER, after all, and we're an indulgent folk!
I also enjoyed the poetry at the end of each chapter. Kind of summed things up.
Over all, this prelude to the final battle is full of suspense and intrigue. I found it haunting and intriguing. Bring on the final battle! I can hardly wait.

CHAPTER 1

A QUESTION OF BALANCE

One of the most interesting discoveries in my consulting work has been the fact that (apparently) every professional service firm in the world has the same mission statement, regardless of the firm's size, specific profession, or country of operation. With varying refinements of language, the mission of most professional firms is:

To deliver outstanding client service; to provide fulfilling careers and professional satisfaction for our people; and to achieve financial success so that we can reward ourselves and grow.

The commonality of this mission does not detract from its value. Simply put, every professional firm must satisfy these three goals of "service, satisfaction, and success" if it is to survive. Management of a professional firm requires a delicate balancing act between the demands of the client marketplace, the realities of the people marketplace (the market for staff), and the firm's economic ambitions.

Many factors play a role in bringing these goals into harmony, but one has a preeminent position: the ratio of junior, middle-level, and senior staff in the firm's organization, referred to here as the firm's leverage. To see the importance of this factor, we shall briefly examine, in turn, its relation to the three goals of the firm.

LEVERAGE AND THE CLIENT MARKETPLACE

The required shape of the organization (the relative mix of juniors, managers, and seniors) is primarily determined by (or rather, as we shall see, should be determined by) the skill requirements of its work: the mix of senior-level, middle-level, and junior-level tasks involved in the projects that the firm undertakes. Consider three kinds of client work: Brains, Grey Hair, and Procedure projects.

In the first type (Brains), the client's problem is at the forefront of professional or technical knowledge, or at least is of extreme complexity. The key elements of this type of professional service are creativity, innovation, and the pioneering of new approaches, concepts or techniques: in effect, new solutions to new problems. The firm that targets this market will be attempting to sell its services on the basis of the high professional craft of its staff. In essence, their appeal to their market is, "Hire us because we're smart."

Brains projects usually involve highly skilled and highly paid professionals. Few procedures are routinizable: Each project is "one-off." Accordingly, the opportunities for leveraging the top professionals with juniors are relatively limited. Even though such projects may involve significant data collection and analysis activities (normally performed by juniors), even these activities cannot be clearly specified in advance and require the active involvement of at least middle-level (project management) professionals on a continuous basis. Consequently, the ratio of junior time to middle-level and senior time on Brains projects tends to be low.

Grey Hair projects, while they may require a highly customized "output" in meeting the clients' needs, involve a lesser degree of innovation and creativity in the actual performance of the work than would a Brains project. The general nature of the problem to be addressed is not unfamiliar, and the activities necessary to complete the project may be similar to those performed on other projects. Clients with Grey Hair problems seek out firms with experience in their particular type of problem. In turn, the firm sells its knowledge, its experience, and its judgment. In effect, they are saying, "Hire us because we have been through this before; we have practice at solving this type of problem."

Since for Grey Hair-type projects the problems to be addressed are somewhat more familiar, at least some of the tasks to be performed (particularly the early ones) are known in advance and can be specified and delegated. The opportunity is thus provided to employ more juniors to accomplish these tasks.

The third type of project, the Procedure project, usually involves a well-recognized and familiar type of problem. While there is still a need to customize to some degree, the steps necessary to accomplish this are somewhat programmatic. The client may have the ability and resources to perform the work itself, but turns to the professional firm because the firm can perform the service more efficiently, because the firm is an outsider, or because the client's own staff capabilities to perform the activity are somewhat constrained and are better used elsewhere. In essence, the professional firm is selling its procedures, its efficiency, its availability: "Hire us because we know how to do this and can deliver it effectively."

Procedure projects usually involve the highest proportion of junior time relative to senior time (and hence imply a different organizational shape for firms that specialize in such projects). The problems to be addressed in such projects, and the steps necessary to complete the analysis, diagnosis, and conclusions are usually so sufficiently well established that they can be easily delegated (under supervision) to junior staff. For Procedure projects the range of possible outcomes for some steps may be so well known that the appropriate responses may be "programmed."

The three categories described here are, of course, only points along a spectrum of project types. However, it is usually a simple task in any profession to identify types of problems that fit these categories. The choice that the firm makes in its mix of project types is one of the most important variables it has available to balance the firm. The choice of project types influences significantly, as we shall see, the economic and organizational structures of the firm.

Consider what will happen if a firm brings in a mix of client work such that its "proper" staffing requirements would be for a slightly higher mix of juniors, and a lesser mix of seniors than it has (i.e., the work is slightly more procedural than the firm would normally expect). What will happen?

As Figure 1-2 suggests, the short-run consequence will be that higher priced people will end up performing lower-value tasks (probably at lower fees), and there will be an underutilization of senior personnel. The firm will make less money than it should be making.

The opposite problem is no less real. If a firm brings in work that has skill requirements of a higher percentage of seniors and a lesser percentage of juniors, the consequences will be at least equally adverse: a shortfall of qualified staff to perform the tasks, and a consequent quality risk.

As these simple examples show, matching the skills required by the work to the skills available in the firm (i.e., managing the leverage structure) is central to keeping the firm in balance.

LEVERAGE AND THE PEOPLE MARKETPLACE

The connection between a firm's leverage structure (its ratio of junior to senior professional staff) and the people marketplace can be captured in a single sentence: People do not join professional firms for jobs, but for careers. They have strong expectations of progressing through the organization at some pace agreed to (explicitly or implicitly) in advance.

The professional service firm may be viewed as the modern embodiment of the medieval craftsman's shop, with its apprentices, journeymen, and master craftsmen. The early years of an individual's association with a professional service firm are, indeed, usually viewed as an apprenticeship, and the relation between juniors and seniors the same: The senior craftsmen repay the hard work and assistance of the juniors by teaching them their craft.

The archetypal structure of the professional service firm is an organization containing three professional levels. In a consulting organization, these levels might be labeled junior consultant, manager, and vice president. In a CPA firm they might be referred to as staff, manager, and partner. Law firms tend to have only two levels, associate and partner, although there is an increasing tendency in large law firms to recognize formally what has long been an informal distinction between junior and senior partners.

Responsibility for the organization's three primary tasks is allocated to these three levels of the organization: seniors (partners or vice presidents) are responsible for marketing and client relations, managers for the day-to-day supervision and coordination of projects, and juniors for the many technical tasks necessary to complete the study. The three levels are traditionally referred to as "the finders, "the minders," and "the grinders" of the business. The mix of each that the firm requires (i.e., its ratio of senior to junior professionals) is primarily determined by the mix of client work, and in turn crucially determines the career paths that the firm can offer.

While the pace of progress may not be a rigid one ("up or out in five years"), both the individual and the organization usually share strong norms about what constitutes a reasonable period of time for each stage of the career path. Individuals who are not promoted within this period will seek greener pastures elsewhere, either by their own choice or career ambitions, or at the strong suggestion of the firm.

This promotion system serves an essential screening function for the firm. Not all young professionals hired subsequently develop the managerial and client relations skills required at the higher levels. While good initial recruiting procedures may serve to reduce the degree of screening required through the promotion process, it can rarely eliminate the need for the promotion process to serve this important function. The existence of a "risk of not making it" also serves the firm in that it constitutes a degree of pressure on junior personnel to work hard and succeed.

Practical and very accessible

This text is a classic resource on the professional service firm, and as a consultant this reviewer appreciates the accumulated wealth of knowledge the author graciously decided to share. The content that Maister provides here is practical and very accessible, and consists mainly of articles published separately over a 10-year time period within such publications as "Sloan Management Review", the "Journal of Management Consulting", "International Accounting Bulletin", and "The American Lawyer". While most of these articles refer to professional service firms as partnerships, the vast majority of the material is still applicable to other types of firms, including the limited liability company (LLC). And the title might suggest a management audience, but the content is important for all associated professionals, including new associates. Because this book is so well written, this reviewer also recommends a reading by those considering work in this field, either those still studying at the university or those contemplating a professional transition. There is no other book quite like "Managing the Professional Service Firm", even though it has been over 15 years since its first appearance (1993) and a host of other texts have entered this space, especially after the late-1990s boom in consulting. The foundation of this book is the simplified mission statement that Maister indicates is the same, with varying refinements, for most professional service firms: "To deliver outstanding client service; to provide fulfilling careers and professional satisfaction for our people; and to achieve financial success so that we can reward ourselves and grow". Probably no coverage of this book can quite deliver justice to its quality. This reviewer set out to list several of the articles which they consider to be above and beyond the rest, but it was difficult to include all but just a small minority of the chapters. The strength of the content is slightly skewed toward the first half of the book, but strong articles persist throughout the 7 sections covering basic matters such as balancing company composition and profitability, client matters such as listening and marketing to clients, people matters such as motivation and scheduling, management matters such as strategy, partnership matters such as performance and compensation, and multisite matters. Highly recommended.

Excellent for Law Firm Managing Partners

I purchased this book out of sheer luck, by reading a review to law firm start up book that recommended professional services firm management books.

I am a East European Lawyer and feel empowered and enlighted when reading this book. Its style is user-friendly. In particular, I liked and appreciated the chapter on what clients expect from professional servicepersons.

I have not yet read all its chapters, but those I did, I read several times and each time new helpfull skills come to my attention.

As a lawyer, new tothis type of source of information, I was not expecting I could find such a book, of such a high quality.
